This form is the first step in our journey together — a space to honor trust, clarity, and mutual respect. This screening process ensures that we are aligned in energy, intention, and safety. It is not about judgment — it is about discernment, integrity, and honoring the space in which you will be.

Complete all fields honestly. Your information is kept strictly confidential and will never be shared. Sessions are offered to those willing to prove their identity and engage in respectful, conscious communication.

Are you a Couple? - Both must fill this form out separately. Once I receive your responses and after I review them, please allow up to 24 hrs for a response. Once verified, a 20-minute call is required for a $50 fee. This goes towards your 1st session when scheduled within 2 weeks. Beyond two weeks, it is a token for my time.

Review my website's Questions page if you do not hear back within 72 hrs for why it may have been denied.

Failure to supply the above information risks this application form being disregarded and thrown in the trash. Your ID must be clear and visible. You may block out the street only. Everything else must be clear and visible.

Cancellation Policy

We understand that life can be unpredictable and that changes sometimes need to be made. Please know that deposits are non-refundable, as they reserve your appointment time. The session type and length determine the deposit.

• If you cancel within 48 hours of your scheduled appointment, 50% of the total fee is due.

• If you cancel within 24 hours, for any reason, the full session fee is required before scheduling future appointments.

• This ensures that reserved time is respected and that scheduling remains fair for all clients.

You agree to pay $50 for your 20 min initial phone consult. Appointments will not be set without this call. 

Clients are responsible for making every effort to attend their appointments. 

By completing and signing this application form, you acknowledge and agree to this cancellation policy. If any payment due under this policy is not made, communication will cease until your debt is paid in full. Payments are accepted through Venmo, CashApp, and major credit cards. CC incurs a 3.5% processing fee.

Appointments after 3 pm on Saturday incur a $100 additional fee.

Should I accept you on a Sunday, at my discretion, there is a $100 additional fee. NO EVENINGS.
